Images in cardiovascular medicine: large unruptured aneurysm in sinus of Valsalva: an unusual cause of right ventricular inflow and outflow tract obstruction.

چکیده
A 64-year-old man with a large cavitating squamous cell carcinoma (636 cm) in the right lower lobe of the lung was referred for prelobectomy cardiac assessment because of incidental finding of Q waves in leads II, III, and aVF on the ECG. Physical examination revealed an ejection systolic murmur at the left sternal edge. Cardiac size was normal on chest radiograph. A transthoracic echocardiogram revealed a large cavity at the aortic root, 5 cm in diameter, obscuring visualization of the right heart (Figs 1 and 2). Diagnostic cardiac catheterization was performed. An aortogram revealed a large (535 cm), unruptured aneurysm in the sinus of Valsalva arising from the right coronary sinus (Fig 3), and a coronary angiogram showed normal anatomy. A right ventricular angiogram demonstrated obstruction at the inflow and outflow tracts of the right ventricle due to compression from the aneurysm (Fig 4). There was a 20 mm Hg pressure gradient across the right ventricular outflow tract. Therefore, a large, unruptured aneurysm in the sinus of Valsalva may be an unusual cause of right ventricular inflow and outflow tract obstruction.

Unruptured Sinus of Valsalva Aneurysm with Right Ventricular Outflow Tract Obstruction and Supracristal Ventricular Septal Defect: A Rare Case.
Unruptured right sinus of Valsalva aneurysm that causes severe obstruction of the right ventricular outflow tract is extremely rare. We describe the case of a 47-year-old woman who presented with exertional dyspnea. Upon investigation, we discovered an unruptured right sinus of Valsalva aneurysm with associated right ventricular outflow tract obstruction and a supracristal ventricular septal de...
متن کاملA Case of Unruptured Aneurysm of the Right Sinus of Valsalva with Right Ventricular Outflow Obstruction
A 66-year-old man presented with exertional dyspnea. He was found to have an unruptured aneurysm of the right sinus of Valsalva causing significant right ventricular outflow obstruction. This aneurysm was diagnosed by transthoracic two-dimensional echocardiography, transthoracic three-dimensional echocardiography, transesophageal echocardiography, contrast echocardiography and 64-slice multidet...
متن کاملBicuspid Aortic Valve: An Unusual Cause of Aneurysm of Left Coronary Sinus of Valsalva
Bicuspid aortic valve is traditionally considered an innocuous congenital anomaly. Due to a better and widespread availability of non-invasive imaging techniques, it has come to the fore that 30% of these cases develop complications, viz., valve abnormality (aortic regurgitation and stenosis), and aneurysm of aortic root and ascending aorta. Sinus of Valsalva aneurysm is an uncommon complicatio...
متن کاملUnruptured congenital aneurysm of the left sinus of Valsalva presenting as acute right ventricular failure.
A patient with unruptured congenital aneurysm of the left coronary sinus of Valsalva presented with acute right-sided heart failure due to right ventricular outflow tract obstruction. The mechanism for such an acute presentation may have been a sudden increase in the size of the aneurysm. The surgical importance of this lesion is the combined aortocameral approach which is seldom required for c...
متن کاملCalcified Unruptured Aneurysm of the Left Sinus of Valsalva
Aneurysm of sinus of Valsalva is a rare anomaly which can be congenital or acquired. The right coronary sinus is the commonest site of aneurysm formation, with only a few cases reported to arise from the left coronary sinus. Although most patients with unruptured aneurysm of sinus of Valsalva are asymptomatic, fatal complications of unruptured aneurysms of sinus of Valsalva have been reported. ...
